Insurance/Club Membership

This form contains the AAU club code for membership and insurance purposes. All athletes MUST have an AAU Membership and be registered with the club in order to participate in practice.

Upon receiving your membership ID, please email your membership card to SCdiveclub@gmail.com.

USA Diving Membership is not required, however if you plan on competing in any USA sanctioned events or at Regionals, Zones, and Nationals, membership will be needed. 

Assumption of Risk & Waiver of Liability Agreement:

This form must be completed before any participation!

Copies are available at the pool. Please ask the coach for one.
